What is neck pain?

Neck pain may radiate to the shoulders, head or arms and may be associated with stiffness, restricted movement, tingling or headache. Duration and distribution help define the pathway.

Not all neck pain is the same.
Diagnosis requires correlation between the clinical history, examination and imaging when indicated.

Causes to distinguish

Posterior joints

Cervical arthrosis and facet joint pain may worsen with extension and rotation.

Nerve root

Disc herniation, protrusion or stenosis may be associated with radiating pain, altered sensation or neurological deficits requiring assessment.

Cervicogenic pain

A cervical source may contribute to occipital pain or headache in selected patients.

Accurate diagnosis

  1. Clinical history and symptom mapping.
  2. Neurological and musculoskeletal examination.
  3. Review of previous MRI, CT or X-rays.
  4. Targeted tests only when they can clarify the treatment target.

A common imaging finding does not by itself establish the cause of pain.

Targeted treatments

The pathway may include education, physiotherapy, medication and, when indicated, injections or radiofrequency treatment of the identified structures. Procedures are planned according to anatomy, symptoms and test responses.

Neurological warning signs require prompt assessment.
Progressive weakness, walking difficulties or changes in bladder or bowel control require urgent medical assessment.
